The City of Midland Public Safety Communications Center is requesting your assistance with gathering information for our first responders. Taking time to fill the forms below will be of service in the case of an emergency for you or a loved one.
Submissions: Please include a current picture of the person missing for any submissions of the Elderly Find Form and Autism & Developmental Disability Registry Form. After completion, fax the completed forms to 432-685-7533 or email to Asa Bishop at abishop@midlandtexas.gov or Angela Wolf at awolf@midlandtexas.gov.
Elderly Find FormThe Elderly Find Form is used for those that have found themselves or someone under their care leaving the house without assistance, often ending up lost or without the means to return home safely. The information provided in this form assists in safely transporting the person of interest back to their residence, or allow for contact to be made with a trusted party. | Autism and Developmental Disability Registry FormThe Autism and Developmental Disability form is used in situations where someone has left the residence in an unknown direction, or someone has been located and unable to communicate with responders. The information provided assists in safely transporting the person of interest back to their residence, or allow for contact to be made with a trusted party. Please include a current picture of the person in your submission. |
Business Key Carrier FormThe Business Key Carrier form is used for businesses, apartment complexes, or other such property managed that would have more information than a primary residence. This information does not overrule information given to an alarm company, but may be used in the event a key carrier or maintenance needs to be reached, for example. This information sheet will help us determine proper resources to dispatch in case of emergency. Please fill out the form entirely and correctly. | Residence & Premise Information FormResidence and Premise Information form is used for primary residences, in situations that additional information that could affect the response of first responders may be needed. This information includes gate/knox box codes, any warning regarding loose or aggressive animals, or information on a resident that needs to be conveyed before a response. This information sheet will help us determine proper resources to dispatch in case of emergency. Please fill out the form entirely and correctly. |
